COVID-19 Self-Assessment

All people entering our offices are required to undertake the following screening.
 
Are you experiencing any of the following symptoms?
 
  • Severe difficulty breathing (for example, struggling for each breath, speaking in single words)
  • Severe chest pain
  • Having a hard time waking up
  • Feeling confused
  • Lost consciousness
  • If over 70, have you experienced:
    • ​delirium
    • unexplained or increased number of falls
    • acute functional decline
    • worsening of chronic symptoms
 
These symptoms may constitute an emergency.  Please call 911, go to your nearest emergency department, or consult
Telehealth Ontario (1 800 797 0000) or the Simcoe-Muskoka District Health Unit (1 877 721 7520)
 
Are you experiencing any of the following symptoms?
 
  • Fever
  • New cough
  • Worsening of a chronic cough
  • Shortness of breath
  • Difficulty breathing (for example, struggling for each breath, cannot hold breath for more than 10 seconds)
  • Sore throat
  • Difficulty swallowing
  • Decrease or loss of sense of taste or smell
  • Chills
  • Headache
  • Unexplained fatigue, malaise, or muscle aches
  • Nausea/vomiting, diarrhea, or abdominal pain
  • Pink eye (conjunctivitis)
  • Runny nose/nasal congestion without other known cause (eg, allergy)
 
Or:
 
  • Have you travelled outside of Ontario in the past 14 days?
  • Have you been tested as positive, or are you awaiting the results of a test?
  • Does someone you are in close contact with have COVID-19 (eg, in your household or workplace)?

A "Yes" to any item above constitutes a positive screening for possible COVID-19. Please follow up with Telehealth Ontario (1 800 797 0000) or the Simcoe-Muskoka District Health Unit (1 877 721 7520), consult your physician, or go to the hospital for further examination.

Please note that, even if the above screening is negative, the risk of attending our offices is increased if:

  • You are over 60
  • You have an underlying medical condition
  • You live with someone who may be particularly at risk

In addition, psychological services generally occur in a small office.  Physical distancing may be difficult, and the time spent together in the space increases the risk of infection, even when masked.
   
If none of the above apply to you, you may receive services face-to-face, provided:
  • Both you and your provider are comfortable with accepting the risk
  • Your provider also passes screening
  • ​Face to face services are essential to the performance of the service
  • The need for the service outweighs the risks posed to the client and the clinician
  • You are willing to wear a non-medical mask
 
Because of the risk posed by asymptomatic transmission, face to face services will be offered only when absolutely necessary, even if the clinician and client pass this screening process.

We ask everyone who attends our offices to wear a cloth face mask, including anyone accompanying the client. Some clients may be unable to wear a mask due to age or presenting issues; please discuss this with our staff.
 
We prefer to offer services face to face, and we hope to return to regular services as quickly as possible.  However, we must place the highest priority on the safety of our clients, staff, and community.
