COVID-19 Self-Assessment

All people entering our offices are required to undertake the following screening.
 

Are you experiencing any of the following symptoms?
 
  • Fever
  • New cough
  • Worsening of a chronic cough
  • Shortness of breath
  • Difficulty breathing (for example, struggling for each breath, cannot hold breath for more than 10 seconds)
  • Sore throat
  • Difficulty swallowing
  • Decrease or loss of sense of taste or smell
  • Chills
  • Headache
  • Unexplained fatigue, malaise, or muscle aches
  • Nausea/vomiting, diarrhea, or abdominal pain
  • Pink eye (conjunctivitis)
  • Runny nose/nasal congestion without other known cause (eg, allergy)
 
Or:
 
  • Have you travelled outside of Ontario in the past 14 days?
  • Have you been tested as positive, or are you awaiting the results of a test?
  • Does someone you are in close contact with (eg, in your household or workplace) either have COVID-19, or are they awaiting the results of a COVID test due to symptoms or close contact with a possible case?

A "Yes" to any item above constitutes a positive screening for possible COVID-19. Please follow up with Telehealth Ontario (1 800 797 0000) or the Simcoe-Muskoka District Health Unit (1 877 721 7520), consult your physician, or go to the hospital for further examination.

Please note that, even if the above screening is negative, the risk of attending our offices is increased if:

  • You are over 60
  • You have an underlying medical condition
  • You live with someone who may be particularly at risk
 
If none of the above apply to you, you may receive services face-to-face, provided:
  • If eligible, you have been fully vaccinated
  • You are wearing a mask 
  • Both you and your provider are comfortable with accepting the risk
  • Your provider also passes screening
  • ​Face to face services are necessary to the performance of the service
  • The need for face-to-face service outweighs the risks posed to the client and the clinician
 
While we respect that both therapists and clients may have preferences for face to face work, we continue to strongly encourage video services whenever possible. Psychological services generally occur in an enclosed space.  The time spent together in the space increases the risk of infection, even when masked. We attempt to reduce this risk through air filtration. However, because the risk of asymptomatic transmission and breakthrough infections cannot be eliminated (and due to the possibility of severe consequences to any infections), face to face services will be offered only when necessary. Necessity may include technological issues (poor internet, poor phone signals), the nature of the service, or the nature of the presenting issue.  

We ask everyone who attends our offices to wear a face mask and sanitize their hands upon entry, including anyone accompanying the client. Some clients may be unable to wear a mask due to age or presenting issues; please discuss this with our staff.
 
We prefer to offer services face to face, and we hope to return to regular services as quickly as possible.  However, we must place the highest priority on the safety of our clients, staff, and community.
